Beginning Friday, August 20th, in compliance with the San Francisco vaccine mandate for indoor dining, State Bird Provisions will require all guests over the age of 12 to provide proof of full vaccination. Outdoor dining is available for unvaccinated guests in limited capacity.

Acceptable forms of vaccination:

– original copy of your COVID-19 Vaccination Record Card

– a photo of your Vaccination Record Card as a separate document

– a digital photo of your COVID-19 barcode with details shown
